We will review the information after receiving your referral. Once we are confident in our ability to deliver safe and appropriate care, we will get in touch with you to further explore your needs and determine what kind of specialised services we can offer.
We will come to your home to assess the risks involved and your unique care needs if you decide to move forward with us. Thereafter, we will draft and cost your care package and present this to you. We are more than happy to speak to relevant and appropriate relatives about the care package and how we can best support you.
We will work closely with service users and their family when designing a care plan.
We will match the most suitable carer to you, ensure they are introduced, and our local team will support you whenever you need support. If you are unhappy with the Carer we have chosen, we will send an alternative person for you to meet.
Following the commencement of your care package, you will receive regular visits from our Care Manager. This is to make sure you are happy with the care we are giving you and to adjust or enhance it as necessary.
